Our Mission – The Bancroft Community Family Health Team is an interdisciplinary team of healthcare professionals working together to provide complete care for all patients.

Our Vision is to ensure residents of Bancroft and the surrounding communities will have access to exceptional primary health care services.

Our Values are focused on the following five areas of strategic importance: 

Patient Centered – We will do our best to work with patients to provide quality healthcare by embracing equity, diversity, and inclusivity.

Integrated Care – We will continue to take a collaborative approach by:  Working with community partners and making healthcare system navigation easier for patients.

Digital Health – BCFHT will continue to expand digital health and virtual care technologies offering remote access and care for patients.

Team Development – BCFHT will provide optimal care while offering training to future and current health care professionals while nurturing a healthy team culture.    

Rural Advocacy – We will continue developing solutions to address the unique needs of our rural setting. Advocating for health systems transformation.
