Registered Nurse – Registered Practical Nurse

Permanent Part-Time Position

 

This 0.4FTE position involves working 2 days per week as an RN / RPN in our High-Risk Foot Care Clinic. Basic and Advanced Foot Care Certification required.

Responsibilities include:

  • Routine nail and skin care
  • Conducting foot health assessments and risk screening
  • Collaboratively identifying priorities for treatment
  • Evaluating the patient’s comprehension and compliance to the foot care plan
  • Providing education to patients
  • Referring to the Chiropodist as necessary.

 

You will be working as a team with our physicians and Family Health Team staff to provide coordinated quality patient care. The ideal candidate must be motivated, able to work independently, and keen to take initiative.

Experience within primary health or acute care settings as well as flexibility to respond to a fluctuating workload and changing environment are important qualities. Proficiency in the use of electronic health records is an asset. Valid registration with the College of Nurses of Ontario as a Registered Nurse or Registered Practical Nurse is required.

Registered Nurse (RAAM Clinic)
Full Time Permanent Position

 This 1.0 FTE position involves working 37.5 hours per week as an RN in our Rapid Access Addiction Medicine (RAAM) Clinic. The RAAM RN will work collaboratively with CMHA HPE Addictions and Mental Health Services to help some of the most vulnerable and marginalized members of our society. The RN’s clinical duties will include conducting patient interviews, recording medical histories, conducting physical and laboratory examinations, and collaborating with our primary care providers and clinic team to establish and assess treatment plans. The role will be part of a multi-disciplinary team, and will also involve liaising with social workers, case workers, and health care professionals, and providing counselling and education to patients and their families regarding addiction, relapse prevention, stress-reduction, and mental health topics.

 

The RN will maintain an up-to-date knowledge of the social determinants of health that create barriers to change for patients using substances and will work with other community partners to address them.

 

Responsibilities:

  • Provide patient centred model of care for those with addictions
  • Deliver a range of harm reduction supports
  • Establish therapeutic relationships with patients
  • Provide community outreach
  • Collaborate with members of our interdisciplinary team to plan, implement, coordinate, and evaluate plans to support the patients with substance use
  • Connect patients with other services in our community
  • Work collaboratively with CMHA HPE Addictions and Mental Health Services to provide coordinated care

Qualifications:

  • Valid registration with the College of Nurses of Ontario as a Registered Nurse
  • Understanding of addictions and substance use and knowledge of harm reduction strategies
  • Awareness and understanding of the unique health care challenges in our rural community particularly the social determinants of health

Registered Practical Nurse
Casual On Call Position

This position involves working called-in shifts and covering illness and vacation leaves for Bancroft Community FHT and the Medical Clinic. You will be working a variety of shifts and days including evenings, weekends, and on-call. The ideal candidate will be positive, energetic, and compassionate.

Duties and Responsibilities

  • Ensuring patient care is provided with established nursing standards and principles, physicians’ orders, and other administrative policies
  • Maintaining accurate clinical records, documented in accordance with CNO Standards as outlined according to protocols, within scope of practice and in accordance with the employer’s policies and procedures
  • Liaising with physician’s/nurse practitioners and other staff to support patients and families
  • Respond to a fluctuating workload and changing environment

Qualifications

  • Current valid registration with the College of Nurses of Ontario as a Registered Practical Nurse is required
  • Excellent communication (verbal and written) and comprehension skills
  • Ability to meet the physical demands of the position
